vid: add cvar vid_minimize_on_focus_loss

Default of 0 matches current SDL default behaviour.

If set, the environment variable SDL_VIDEO_MINIMIZE_ON_FOCUS_LOSS will
override the cvar.

Changes apply immediately.

Also makes changes to vid_mouse_clickthrough apply immediately.
