changes to rtlight surface culling:
SVBSP culling mode now also checks PVS (not sure why this helps)
no longer rejects backfaces in light culling (fixes several glitches in
start.bsp and elsewhere - not sure why the light would be considered
behind these triangles though)
optimized svbsp building conditions a bit
